Deadline check: The AI Omnibus moved application of high-risk rules for Annex III systems to 2 December 2027 and high-risk systems embedded in Annex I regulated products to 2 August 2028. Do not treat 2 August 2026 as a universal Annex IV deadline. Check your system's classification, role and transitional rules. European Commission update.

What the nine parts ask for

Article 11 calls for documentation before a high-risk system is placed on the market or put into service, kept up to date, containing at least the applicable elements in Annex IV. A questionnaire can organize the work; it cannot verify your claims or replace underlying evidence.

Read the consolidated regulation and the Commission's Annex IV reference. For product-linked systems, Article 11 can require one documentation set including relevant sectoral-law information. An SME or small mid-cap may be eligible for a simplified form under the amended Article 11; this tool does not claim to be that official form.

Build your working draft

Write specifics and cite your own evidence in the answers: versions, owners, dates, test reports, datasets and links. Write "Not applicable - because..." where justified. Empty answers remain flagged in the export. No account or upload is needed.

Export any time. The HTML file can be printed to PDF in your browser. Review the draft with legal, safety and technical owners before relying on it.
